The AJT Academy Community Camp is a free community event hosted by the AJ Terrell Jr. Foundation at Westlake High School on May 9 from 8:00 AM–2:00 PM. The camp welcomes students in grades 1–6 for a fun, introductory flag football experience led by AJT Academy coaches and high school student-athletes from Westlake and Frederick Douglass.
While campers participate in drills, games, and team activities, families are invited to explore a community wellness fair featuring local organizations providing, samples from local restaurants, health screenings, resources, and information that support youth and family wellbeing. The event is designed to introduce younger students to sports, mentorship, and positive role models while connecting families with valuable community resources.

Guardian Authorization Statement
By completing and submitting this registration form, I certify that I am the parent or legal guardian of all participants listed in this registration. I affirm that I have the legal authority to register the participant(s) for the AJT Academy Community Camp and to execute all required permissions, waivers, and releases on their behalf.
I acknowledge that the information provided is accurate to the best of my knowledge and that I am authorized to act as the participant’s legal representative in matters related to their participation in this event.
Name, Image, and Likeness Release By registering my child for the AJT Academy Community Camp, I grant permission to the AJ Terrell Jr. Foundation and AJT Academy to photograph, film, or otherwise record the participant during camp activities.
I authorize the AJ Terrell Jr. Foundation, AJT Academy, and their partners and sponsors to use the participant’s name, image, likeness, voice, and appearance in photographs, video recordings, or other media captured during the event for lawful promotional, educational, and marketing purposes. This may include use in social media, websites, printed materials, media coverage, sponsor communications, and other promotional content related to the AJ Terrell Jr. Foundation, the AJT Academy, the Community Camp, and affiliated sponsor initiatives.
I understand that these materials may be used without further notice or compensation and may be distributed publicly in perpetuity.
By completing this registration, I certify that I am the parent or legal guardian of the participant and that I grant this permission on their behalf.
Liability Waiver & Release I acknowledge and understand that participation in athletic and recreational activities, including flag football drills and games, involves inherent risks that may result in accidents, injury, serious bodily harm, or death.
In consideration for my child being allowed to participate in this event, I, as the parent or legal guardian of the registered participant, voluntarily assume all risks associated with participation in the AJT Academy Community Camp.
I hereby release, waive, and hold harmless the AJ Terrell Jr. Foundation, AJT Academy, Westlake High School, Atlanta Public Schools, Fulton County Public Schools, and all organizers, sponsors, volunteers, staff members, partners, and affiliated individuals or entities involved in the planning or execution of the camp from any and all liability, claims, demands, causes of action, damages, losses, or expenses arising out of or related to any injury, illness, accident, disability, or death that may occur during or as a result of participation in the event.
I certify that the participant is physically able to take part in the activities offered at the camp and that I understand and accept the risks involved. By completing this registration, I confirm that I am the parent or legal guardian of the participant and that I agree to the terms of this waiver and release.
